Preparing for Your Appointment
Wait area overlooking the reception at Advanced Center for Oral Surgery, Facial Surgery and Dental Implants.To assist us in providing you with the best possible care, please bring the following with you on the day of your appointment:
•  A referral slip from your doctor.
•  A list of medications you are presently taking.
•  A government issued photo ID.
•  Complete insurance information including the member’s name, social security number, date of birth and name of the employer.
•  Minor children under the age of 18 must be accompanied by a parent or legal guardian.

Please arrive 10-15 minutes early for your first appointment to complete your registration with our office. To expedite your first visit, please complete our online registration form and health history by clicking the link below.
